      Measurement for improvement is an important part of participation in the TOP Collaborative. Measurement tells us if we are improving and reaching our aims, helps teams communicate about their improvement efforts, and ultimately demonstrate the impact on the community.

      faculty has developed two resources: a Driver Diagram (a graphic to illustrate factors that contribute to the
      overall TOP aims) and a Change Package (a listing of interventions for consideration by TOP teams to test in
      working towards reaching their individualized aims).

      The Plan-Do-Study-Act (PDSA) cycle is a useful tool for documenting a test of change. Running a PDSA cycle is another way of saying testing a change- you develop a plan to test the change (Plan), carry out the test (Do), observe, analyze, and learn from the test (Study), and determine what modifications, if any, to make for the next cycle (Act).
