Discharge Summary Complex Case Management Form – IDC

This form provides a brief summary of care provided to the client by our team as well as a snapshot of the client’s health status pre and post discharge to the team.  The team has found this tool to be helpful as it serves as a “double check” prior to discharge that all major health needs of the client have been met.
